[ note: click your mouse outside any form fields - if a cursor is blinking inside a field when the email button is clicked, the text entered in that field will not be saved. ]  
 
2.)  Save the form to your computer by clicking on the HERE link shown above with your right mouse button and selecting "Save this Link As." Make note of where you save the file.  You may then locate the file and open the form to fill it out on your computer.  Once completed you may either click the "Email to Registrar" button found on the form .. or ..  print the completed form in order to hand deliver or mail it to the church office at
 

3.)  Print the form from the internet by going to FILE on your internet browser's toolbar and selecting print. You may then fill it out by hand, and hand deliver or mail the completed form to the church office at the address above.
