Choices for Fillable PDF form submission include:

1.) Fill out the form online and click the "Email" button found on the form to email it to its recipient.

      [note: click your mouse outside any form fields - if a cursor is blinking inside a field when the
               email button is clicked, the text entered in that field will not be saved.]  

2.) Save the form to your computer.  Make note of where you save the file.  You may then locate the file and open the form to fill it out.  Once completed save the changes and you may either click the "Email" button found on the form to email it .. or ..  print the completed form in order to hand deliver or mail the form to the church office.

3.)  Open and print the form from the internet by going to FILE on your internet browser's toolbar and selecting print. You may then fill it out by hand, and hand deliver or mail the completed form to the church office.
