Calendar Request Form

This calendar
request form is a fill-able Adobe PDF document.
Choices for submission include: |
* * Open the form by clicking on
the Calendar Request Form button shown above.
Fill out the form online and click the "Email this Form"
button found at the top of the form to email it to the
church office.

note:
click your mouse outside any form fields - if a cursor
is blinking inside a field when the email button is
clicked, the text entered in that field will not be
saved.

* * Save the form to your
computer by clicking on the Calendar Request Form
button shown above with your right mouse button and
selecting "Save this Link As." Make note of where you save
the file. You may then locate the file and open the
form to fill it out. Once completed save the changes
and you may either
click the "Email this Form" button found at the top
of the form to email it .. or .. click the "Print Form"
button at the top of the form to print the
completed form in order to hand deliver or mail the form to
the church office.
* * Print the form from
the internet by going to FILE on your internet
browser's toolbar and selecting print. You may then fill it
out by hand, and hand deliver or mail the completed form to
the church office.
